Please fill out the application and return by one of the following methods.
Drop off at the Pike County Senior Center in person, 8:00am to 4:30pm M-F.
Use the black drop box at front entrance of the Pike County Senior Center.
Email to mlucas@pikecac.org.
On your first pick up, bring the application.
Blank applications will be available at the sites on distribution days.
